I'm thinking the cooling efficiency could be improved by also placing a small low RPM fan on the thin metal slab but those obstructive heatpipes will warrant a custom fan design. I've seen some crazy cooling designs in Dell office PCs but nothing could've prepared me for this. Someone thinks they are very clever or it does work great.
 
Should work ok as long as it's oriented correctly. It won't shatter any records but for a standard business PC or something that would be more than adequate. That said, not exactly easy to install or pull off if you needed to repaste, and very specific to that motherboard/case combo.

I think your idea of putting a fan on the metal may be counter-productive though. You need the heat pipes to be able to conduct up to the fins, and if you are cooling them there, they won't flow up as well.
 
I agree with @Brian_B, it should be adequate in its given setup, but not amazing, and very specific. If it were possible to make the heatpipes flexible I could see something like this working.
